MSW Application Procedures

Step 1: All applicants must complete the online application by clicking the link below
Applicants must create an account and follow the initial application instructions
Be sure you indicate that you wish to apply for the Master of Social Work Program.
Step 2: Additional MSW Supplemental Application Material
Applicants will receive an email with further application instructions from the USU School of Graduate Studies.
Applicants will be asked to download, complete and upload the following documents in a pdf format

The online application will also request the completion of a form concerning your statement of purpose to ascertain your interest in, understanding of, and experience related to social work as a profession.  Please note that note that this form is compatible with the most recent versions of Google Chrome, Mac IOS Safari, and Adobe Acrobat. The application process requires a resume with relevant research, work and volunteer experience.  Please follow the instructions included with the attached form and submit the form to your application at You may download a copy of this form here.

Step 3: MSW Letter of Reference

Applicants will enter contact information for three recommenders in the Graduate School's system.
Recommenders will be contacted by the Graduate School.  
Please note that recommenders must use the specialized fillable pdf form to provide their recommendations. 

Three satisfactory letters of recommendation are required for the completion of any MSW application. Ideally, one letter will be submitted for each applicant from a faculty member familiar with the applicant's academic work, a field supervisor from a practicum or internship experience, and a supervisor from a significant human services work or volunteer experience.  If the applicant will be graduating with a BSW this coming semester, a letter from his/her practicum/internship supervisor is required for consideration into the Advanced Standing program.

The School of Research and Graduate Studies will send each recommender an electronic form to complete, as identified by the applicant. Please note that this form is compatible with the most recent versions of Google Chrome, Mac IOS Safari, and Adobe Acrobat. Please direct your referees to complete this form through the associated directions that are emailed to them through the School of Research and Graduate Studies. You may download a copy of this form here.
Step 4: Provide a copy of all your official academic transcript
All transcripts are to be sent directly to USU's School of Graduate Studies
Use the Graduate School's transcript code: 4857 or email:

Applicants are required to submit official transcripts for each college or university attended. Please have the transcripts sent to the USU School of Graduate Studies Office. (USU Transcript code: 4857)

Applicants who have graduated from USU do not need to submit a transcript.

Step 5: Register to take the GRE or MAT admissions test  or submit a portfolio

Results to be sent directly to the USU's School of Graduate Studies

Entrance Exam:  Either the  GRE (Graduate Record Examination) or MAT (Miller Analogies Test) is a prerequisite for admission. Both tests are equally acceptable.  If you have taken an entrance exam multiple times, the highest score will be used in the application process. When submitting your application, you may record unofficial test scores, but official test scores must be reported directly to the USU School of Graduate Studies.  Applicants must demonstrate a score or scores at or above the 40th percentile of the above stated exams.  

Portfolio Option:  Applicants with two or more years of paid human service work experience by the application deadline may submit a portfolio in lieu of the entrance exam. Portfolios should include include the required form with a rating of their current level of competency in nine areas and a one-page letter addressed to the MSW admissions committee that describes their human service experience and the scope of their responsibilities.  Portfolios can be submitted for review as a supplemental item after the application fee has been paid. Download the Portfolio Option Form here.
